Between-Visit Care Isn't a Convenience. It's Where Recovery Is Won — and Where Danger Is Caught Early.

Remote Therapeutic Monitoring · Evidence Summary for Policymakers

Patients recovering from injury and surgery spend fewer than one percent of their recovery time in a clinic. Remote Therapeutic Monitoring (RTM) closes that gap with daily, clinician-reviewed check-ins—and more than three years of real patient feedback, alongside validated clinical outcomes, shows what that gap-closing does for patients, for providers, and for the health system that depends on both.

12,134

Improvement across validated functional outcome measures

Fewer in-person visits needed to reach the same goals
  • Part One: Patients love it — Satisfaction, compliance, and perceived value of care all rise when patients have a daily line to their care team.

  • Part Two & Three: It saves lives — Validated outcome measures improve across the board—and early detection turns delayed diagnoses into same-day care.

  • Part Four: Providers thrive — Clinicians report deeper connection to patients and more confidence in the care they deliver between visits.

Part One — The Patient Experience (N = 12,134)

Patients Say It Makes Them Feel Safer, and Cared For

This is not a small pilot. Over three years, 12,134 patients responded directly to SaRA Health's patient-experience survey—and the pattern holds at scale: daily, low-effort check-ins keep patients accountable to their recovery and make them feel genuinely looked after—not "monitored."

76%
Say check-ins improved their overall therapy experience
80%
Say the check-ins increased the perceived value of their care
58%
Report improved compliance with their home exercise program
73%
Say the daily frequency of contact is "just right"
83%
Say their responses had a real, positive impact on the quality of their care
72%
Say the check-ins successfully kept their provider informed between visits

What patients are saying:

"It helps me be more self-aware of specific symptoms and pain… it greatly supports me to intentionally tend to my pain management, which is valuable not only for that but for my overall well-being." — Patient, Patient-Experience Survey

"I've been using these people for 15 years… I tell everyone I know about the great care I receive." — Patient, Patient-Experience Survey

"It was like having a close friend or workout buddy checking in every day… imagine if other doctors or hospitals would utilize this." — Patient, Patient-Experience Survey

"The team is very helpful in helping me understand absolutely everything… I couldn't have done it on my own." — Patient, Outpatient Rehab

"We always feel welcomed and treated with respect, and desire to help improve our strength!" — Patient, Outpatient Rehab

"I am very satisfied with the existing messaging service and would keep it as is. The care I receive is excellent." — Patient, Patient-Experience Survey

Why it matters: This holds up at real scale, not just in a small pilot. Across more than twelve thousand patients, engagement and perceived care quality move together—the mechanism that keeps patients on their plan of care and prevents the setbacks that drive avoidable utilization later.

Part Two — Clinical Outcomes

The Functional Data Backs Up the Patient Stories

A controlled outcome study compared 260 patients enrolled in RTM against 220 similar patients receiving standard care, across four validated functional outcome measures used industry-wide to track recovery.

Functional outcome score improvement, RTM vs. standard care
Raw score change from baseline to discharge (higher = better recovery); Standard care n=220, With RTM n=260
Knee Outcome Survey (KOS)+23.4%
Lower Extremity Functional Scale (LEFS)+30.8%
Disabilities of the Arm, Shoulder & Hand (DASH)+5.0%
Neck Disability Index (NDI)+11.8%
Standard care (n=220)
With RTM (n=260)

Fewer patients fall short of meaningful recovery RTM patients were far less likely to miss the minimal clinically important difference (MCID) on every measure—a 36–62% reduction in "failure to progress" rates across KOS, LEFS, DASH, and NDI.

Better outcomes, delivered more efficiently RTM patients needed 2.3 fewer visits to reach comparable or better results, with functional scores averaging 11.5+ points above national benchmark estimates—recovery that is both better and lighter on the system.

Why it matters: These are the same validated instruments used to judge rehab quality across the industry—not a proprietary metric. Better scores on fewer visits means better recovery and less total utilization, not a tradeoff between the two.

Part Three — Catching Problems Before They Become Emergencies

This Is What Early Detection Looks Like in Practice

The core premise of RTM is simple: a clinician who sees a warning sign on day 3 can act on day 3, instead of finding out at the next scheduled visit—or in an emergency room.

Documented case · Reported by a treating clinician

A patient recovering from surgery was completing routine daily check-ins through their RTM program. One response stood out from the pattern—language suggesting unusual shortness of breath. The system surfaced it immediately to the treating clinician, well before the patient's next scheduled visit.

The clinician recognized the warning sign, reached out, and got the patient into care the same day. The diagnosis: fluid on the lungs, caught early enough to treat directly—avoiding an emergency room visit and the more invasive, more costly procedures that a delayed diagnosis would have required.

This is the mechanism, not the exception: every RTM check-in is a chance to catch a problem while it is still small.

Part Four — The Provider Experience

Providers Feel Closer to Their Patients — and More Confident in Their Care

73%
Of clinicians agree RTM effectively alerts them to warning signs of patient decline before it becomes an adverse event.
78%
Of clinicians say RTM makes them feel more connected to their patients' recovery between visits.
Independent, peer-reviewed-track research confirms it n=43 pilot
Clinician confidence in RTM's early-warning detection is significantly correlated with stronger therapeutic alliance with their patients. Feeling more connected to a patient's journey between visits is strongly correlated with an overall positive experience using the platform—and clinicians with the most positive experience are significantly more likely to report that RTM lets them manage patients proactively, rather than reactively.

Why it matters: Rehab therapy is contending with real workforce strain. A tool that makes clinicians feel more connected to their patients, not more burdened by paperwork, is a retention lever as much as a clinical one—and retention is a policy problem, not just a staffing one.

The Case for Continued Support

Why Policymakers Should Keep Investing in RTM

The evidence points the same direction from every angle—patient, clinician, and system. That consistency is the strongest argument for keeping Remote Therapeutic Monitoring codes intact, and for expanding what they can do.

  1. Patients want this. Satisfaction, compliance, and perceived value of care all rise when patients have a daily, low-friction channel to their care team.

  2. It measurably improves recovery. Every functional outcome measure studied moved in the right direction, with fewer visits required to get there.

  3. It catches danger early. Between-visit monitoring turns a delayed diagnosis into a same-day intervention—before it becomes an ER visit.

  4. It strengthens the workforce delivering care. Clinicians report feeling more connected to their patients and more confident in the care they're providing—at a moment when rehab therapy is contending with real workforce strain.

This is the story behind PTsarelifesavers.com

A campaign built on one idea: therapists who stay connected to their patients between visits don't just improve outcomes on paper—they save lives. RTM reimbursement codes are what make that connection possible at scale.
