RTM denial help
Get help with an RTM denial.
Tell us which payer denied which codes and the denial reason codes. Someone on our billing team will reply by email with what likely went wrong and how to fix it, whether or not you use SaRA.
Send us the denial
Send only the payer, the CPT codes and the denial reason codes. No patient names, dates of birth, member IDs, claim numbers, dates of service or EOB attachments.
Common causes
What we see most often
Billed as a visit
RTM codes are monitoring and management services, not visits, and have their own rules.
Date of service
Each RTM code has its own date-of-service rule, and payers differ. A date that doesn't fit the code's period is a common denial.
Monitoring days
98977 needs 16 to 30 days of data in a 30-day period; 98985 covers 2 to 15 days. Only one of them per period.
Documentation
Consent, monitoring days, clinician time and the monthly live conversation all need a record.
